What is Revision Knee Replacement?

Though Knee Replacement is one of the most safest procedures and lacs of patients have benefited from it world-wide, over a period of time, it may fail due to multiple reasons and require a revision knee placement to be done.

A replacement surgery is expected to last a long time. The longevity of the replacement surgery is called survivorship. Survivorship indicates how many patients have had their replacement re-done or revised due to any cause.

When all the factors for revision surgery are taken into account- infection, aseptic loosening or wear etc, the survivorship of the replacement surgeries is about 95-98% at 10 years and 90-95% between 15-20 years.

That means it is likely that about 5% of people undergoing a replacement may require a revision surgery within 10 years of the first replacement.

What Causes Knee Replacement to Fail ?

Knee replacement can failure due to below reasons

1. Infection
2. Trauma
3. Instability (loose ligaments holding the knee stable)
4. Implant dislocation
5. Fractures
6. Stiffness

People with failed knee replacements may experience pain & swelling. Their knee may lose flexibility and become stiff, making it difficult to carry on with daily activities. 

Revision Knee Replacement Procedure

There are different types of revision surgery. In some cases, only one implant or component of the prosthesis has to be revised. Other times, all three components—femoral, tibial, and patellar—need to removed or replaced and the bone around the knee needs to be rebuilt with metal components or bone graft.

In patients with an infection the revision needs to be done in 2 stages. In patients with loosening without infection the surgery is done in a single stage.

Two Stage Revision

In patients with infection of the joint the revision is called a two-stage revision. In the first stage the previous implants are removed. The infected tissues are all cleared, this process is called debridement. Once the infected tissues are cleaned the tissue samples are sent for testing to find out the exact bacteria responsible for the infection. This allows the best antibiotic combination to be given for the patient to get infection clearance. Antibiotics may be required for 4-6 weeks.

Once the infection is cleared then the second stage involves putting in a new implant.
In case of revisions, the bone structure on which the implant can be fixed, or the ligaments which hold the tension in implants may have lost their strength and tension. This necessitates the use of special implants which can compensate for the loss of bone and ligament strength.

In revision knee replacements the bone loss can be managed by use of augments, metaphyseal sleeves, tantalum trabecular metal cones and stem extensions for both the femur (thigh bone) and the tibia (leg bone). Sometimes the ligaments are completely redundant, and these patients may require the use of Hinged implants.

What happens after the surgery?

Recovery after revision knee replacement might be slower and you may have to stay in hospital for a few days. Four critical points have been listed below.

Pain Management : You might feel some pain and it is part of healing process. Dr. Venkatdeep and his team will provide required medications to manage your pain.

Blood Clot Prevention : You may be given blood thinners and asked to wear a special stocking. An inflatable sleeve could be placed around your calf which will compress intermitently to improve blood flow.

Physiotherapy : Our physiotherapist will guide you to do certain exercise's and will help you to walk and resume normal activities.

Infection prevention : Depending on your case, you might be prescribed an antibiotic to prevent infection.
